Build strong relationships with patients to encourage consistent participation in RPM programs.
Conduct scheduled follow-up calls with stable patients twice per month (every 2 weeks).
Answer patient questions related to devices, readings, and general health concerns.
Provide friendly, professional, and patient-centered support—especially for elderly patients.
Educate patients on their health conditions, medications, and care plans as defined by providers.
Guide patients in adopting healthier behaviors (diet, exercise, lifestyle improvements).
Assist patients with device setup and usage (e.g., BP cuff placement, proper measurement techniques).
Provide basic troubleshooting support for RPM devices; escalate when needed.
Reinforce best practices (e.g., resting before BP measurement, correct positioning, consistent timing).
Act as a liaison between patients and healthcare providers/clinics.
Escalate abnormal readings, symptoms, or concerns to the RN/Provider in a timely manner.
Recognize urgent symptoms (e.g., signs of heart attack or stroke) and instruct patients to contact emergency services immediately.
Ensure proper follow-up and continuity of care for each patient.
Accurately log all patient interactions, time spent, and observations in the EasyLinkMD system.
Maintain clear, structured, and timely documentation for compliance and billing purposes.
Ensure strict adherence to HIPAA and patient confidentiality standards.
